There are only 3 fragments actually and you take them to an area south east of the main way-point or south from where you obtain the skull of arvak, then when inside said area you place all the pieces on the pedestal and it'll summon the reaper who will drop 3 black soul gems and an iron great-axe this is just what type of weapon he dropped for me and my friend so i'm not sure if it's the same for everyone.
